General Grievous in his four armed configuration from the Utapau duel, one of the Deluxe releases from the original 2005 Revenge of the Sith line.
Includes:

The Secret Lightsaber Attack feature splits each arm at the shoulder so the figure can hold all four lightsabers at once. Released on the red and black ROTS Deluxe card under assortment 85182, alongside the Obi-Wan Kenobi with Force Jump Attack, Chancellor Palpatine and Anakin Skywalker deluxe releases shown on the cardback.
